Job Summary

Reporting to the Child protection and Case Management Officer, the incumbent(s) will be responsible for supporting case management of all beneficiaries enrolled for the OVC program. Their roles will include, profiling and enrolment, assessments, care plan development and reviews, service monitoring, benchmark tracking, home visits, day to day management of the case files as well as case closures and post program graduation follow ups.

Duties and Responsibilities

Responsibilities
• Supporting the roll out of the case management process in providing services and to inform case managers of the necessary services each client needs to be offered.
• S/He will conduct Household profiling and assessments to determine eligibility for enrollment of beneficiaries into the OVC program.
• S/He will develop care plans in consultation with households, monitors care plans implementation, review care plans as needed from time to time.
• Conduct - case follow-ups for every client according to the Case Management SOP and to do so every quarter according to the PEPFAR guidelines.
• S/He will conduct Household benchmark assessments to beneficiaries to ensure that services are reaching the intended beneficiaries and their needs have been met (graduation)
• S/He will monitor service provision of all beneficiaries to ensure that the goals of the care plans as agreed have been met.
• Documentation of beneficiary information and record keeping as guided by the HOSPAZ Case Management SOP.
• Assisting with the support of broader organizational activities in child protection and case management.
• S/He will ensure a coherent filing system and ensuring that all files have the needed contents.
• Through the case management cycle compile case updates for all beneficiaries to show any change in circumstances of a beneficiary.

Qualifications and Experience

Qualifications and experience
• Bachelor’s degree in Social Work.
• Attachment experience with a reputable organization.
• Registered with the Council for Social Workers in Zimbabwe.
• Should be able be highly productive with minimal supervision.
• Knowledge of technical areas such as HIV, SGBV, child protection, work with adolescents.
• Should be a team player.
• Should have good communication skills.
• Should be flexible to work weekends; after hours and be on call during emergencies.
• Class 3 driver’s license will be an added advantage.
